What this is

A structured screen, not a diagnosis.

This tool helps a teacher, parent, clinician, or learner gather structured information about learning across several domains, turn it into a clear profile, and produce evidence-informed support recommendations and a printable report.

Important. This is a screening and planning aid only. It does not diagnose dyslexia, dyscalculia, ADHD, or any other condition, and it is not a substitute for a formal evaluation. A diagnosis can only be made by a qualified educational psychologist or specialist using validated, norm-referenced instruments. Use the results to decide whether to seek a full evaluation and to plan immediate classroom and home support.

Items shown are starter behavioural indicators drawn from common signs of learning difficulty, filtered to the learner's age band. They are illustrative and should be reviewed, adapted, and validated for your context before institutional or research use.

Step 02

Intake & background

Identify the learner and the person completing the screen. You may keep the learner anonymous by using a coded ID instead of a name. The age sets which items appear.

Step 03

Screening all ages

Rate how often each statement is true compared with others of the same age. Skip an item only if you truly cannot judge it. There are no right answers — describe what you observe.

Learning profile

Each bar shows the proportion of difficulty indicators reported in that domain. Markers show the thresholds above which closer attention is suggested.
